How well-mannered is your dog?

1. I would like to train my dog because:
I just adopted him
I'd like to strengthen our relationship
He's got some behavioral problems
2. Is your dog crate trained?
Yes, he's comfortable in a crate
No, my dog doesn't like his crate
What's a crate?
3. How many housetraining accidents does your dog have?
None
One or two a month
More than two a week
4. Does your dog destroy shoes, furniture, or other stuff that's off-limits?
No, or rarely
Now and then
More frequently than I like to admit
5. What does your dog do when left home alone?
Sleeps or chews a toy
Barks, chews something he shouldn't, or raids the garbage
Seems stressed, relieved, or exhausted when I return
Won't eat anything until I return home
Tries to escape, destroys his surroundings, or harms himself
6. How does your dog act when walking on leash?
He's respectful of the leash, or pulls only occasionally
He hauls me around town
He barks or lunges at passersby or other dogs
7. How does your dog behave when meeting new people?
He greets them happily
He takes a while to warm up -- if ever
He growls, barks, or stiffens
8. If your dog has bitten anyone, how serious was it?
It was more of a snap and didn't leave any marks 
It left a mark, but didn’t bleed or require medical attention
It drew blood and/or left a puncture wound
