At 4 months the bladder does not have a lot of “muscle” control so any sort of excitement/stimulation/fear can cause a release.
What I would do is IF you know people are coming over, take him out for pee just before they arrive to reduce the amount of urine in his bladder.
For expected OR unexpected guests the best I can suggest is to not give him any attention for the first five minutes the people are there. Ask them not to pat, talk to, or engage in any way with the pup until he is calm. You will find this benefits many things in the future as well, including jumping on people!
Also work on teaching him a strong sit. He should learn to sit before he gets attention, food or any other type of “reward”.
It will take some time for it to be solved, his bladder needs to mature so just be aware that it he really cannot help himself at this point and the calmer you are the calmer he will be.

From what i know, Incontinence (uncontrolled bladder)can affect dogs of either gender, but it typically doesn’t occur until the middle or later years of the dog’s life. And since yours is still on its baby stage,i sure hope he doesn’t have it.

But just to be sure, here’s a link that you can visit to give you some good information about dogs’ natural peeing behavior and incontinence:
